A rare thrombotic disease with a frequency of 1 to 10 people per million people can appear after AstraZeneca (AZ) Corona 19 vaccination, according to the Expert Committee on Vaccination. It was considered that this is the case in the 20s where the formation of blood clots was reported after the recent AZ vaccination. At the same time, the expert committee emphasized, “There is no reason to avoid vaccination because of blood clots because it is a very rare disease and it can be healed if you receive treatment before it gets worse.”

In addition, out of 10 cases reported as suspected anaphylaxis or severe adverse reactions, two were judged to be related to vaccination. This is the first time the government has officially recognized the causality between vaccination and severe adverse reactions.

The most interesting cases are the first responders of Corona 19, who reported the formation of blood clots after AZ vaccination, and the residents of nursing hospitals in their 60s, whose blood clots were confirmed at autopsy after AZ vaccination and death report. Among them, in their 60s, deep vein thrombosis and pulmonary thromboembolism, which are caused by clots blocking blood vessels, were confirmed. In Europe, the number of common thrombotic diseases after vaccination between AstraZeneca and Pfizer was similarly reported. However, Eunhwa Choi (Professor of the Department of Pediatrics at Seoul National University) said, “It is very unlikely that the vaccine caused blood clots in people in their 60s.”

In contrast, in their twenties, they were diagnosed with cerebral sinus thrombosis (CVST). In Europe, 18 cases of CVST and 7 cases of disseminated intravascular coagulation disorder (DIC) were reported among more than 20 million vaccinations. CVST is a specific case that is different from the general disease caused by blood clots. The spontaneous incidence rate is very rare, ranging from 1 to 10 per million. Chairman Choi said, “Overseas, after AZ vaccination, CVST is higher than expected,” he said. “The causal relationship is not yet clear.”

Women under 55, need more care after vaccination

CVST is very rare, but preparation is needed as more inoculations can increase the likelihood of occurrence. CVST is accompanied by severe headaches or blurred vision as blood clots block blood vessels in the brain (venous sinuses), which increases brain pressure. The expert committee recommended that if you take painkillers after vaccination and do not get better for more than three days, you should take tests such as brain computed tomography (CT) and magnetic resonance imaging (MRI).

In addition, if you are easily bruised on your limbs after vaccination or even a small impact, or if you see red dots of about 2~3㎜ in length, you should suspect DIC. Even at this time, it is better to find a large hospital that can control bleeding and treat multiple growth organ dysfunction.

As the European Medicines Agency (EMA) reports that relatively more thrombotic abnormalities are reported in women under the age of 55, young women need to be more concerned about the symptoms after vaccination. An official from the Ministry of Food and Drug Safety said, “We have requested information on abnormal symptoms from countries around the world from AZ.”

To date, 16 cases have been reported in Korea after being vaccinated against Corona 19. The expert committee denied causality with vaccines in 13 of these cases. The remaining three are under autopsy or deliberation. In addition, two out of 10 cases reported as suspected anaphylaxis or severe adverse reactions were recognized for causality with the AZ vaccine.

This is a case of a patient in a medical institution in his 20s who showed symptoms of anaphylaxis 7 minutes after vaccination, and a patient in a nursing hospital in his 40s who showed symptoms of convulsions and high fever 12 hours and 15 minutes after getting the vaccine. Now, all symptoms have improved. “If causality is confirmed, you can receive medical expenses, examination expenses, and nursing expenses according to the national damage compensation procedure,” said Eun-kyung Jung, head of the vaccination response promotion manager.

Nevertheless, the expert committee concluded that the AZ vaccine should continue because the benefit from vaccination is far greater than the risk of adverse reactions. “Assuming that the number of confirmed cases is 100,000, the vaccine is effective at 80%, and the mortality rate is 2%, assuming that the number of confirmed cases is 100,000, and the mortality rate is 2%, more than 1,500 deaths can be reduced, but the probability of dying from adverse reactions such as CVST is One person,” he said, and said, “Because the benefits of vaccination are much greater, please get the vaccine for safety.”
