Gyeonggi Province Governor Lee Jae-myeong said on his Facebook page on the 23rd that “in an emergency, nurses should be allowed to vaccinate against Corona 19.” This article is aimed directly at the Korean Medical Association. The Korea Medical Association recently rebelled against the’medical law amendment bill’, which is the key to canceling the license of a doctor who was sentenced to a prison sentence or higher and put a strong strike.
The intention is to ensure that the nurse is responsible for the pre-vaccination step so that even if a general strike of doctors across the country as claimed by the medical association does not interfere with the corona 19 vaccination. Governor Lee wrote, “If an illegal strike by the National Assembly becomes a reality, the lives of 13.8 million residents will be exposed to danger.” Is it possible to leave the vaccination entirely to the nurse as Governor Lee argues?

Move as a doctor/nurse’one team’

At the time of vaccination, doctors and nurses are’one team’. Chung Kyung-sil, head of the vaccination management team of the Corona 19 Vaccination Response Promotion Team, said at a regular briefing on the 23rd, “After a doctor predicts, a nurse will inoculate according to the results.” According to the Central Vaccination Center, those who are eligible for the COVID-19 vaccine fill out a preliminary form according to the nurse’s guidance. Write down the pregnancy, allergies, underlying diseases (internal diseases), and the presence or absence of medication. Based on this, a doctor’s interview is made. Examine the correlation between chronic disease and vaccine, and examine the subject’s physical condition. It is up to the doctor to make the final decision on vaccination. Vaccination is done by a trained nurse.

It doesn’t end here. The inoculum will stay in the hospital for 15 to 30 minutes. To see if there are any adverse reactions. If the vaccinator shows any adverse reactions, first aid is provided by a doctor. Governor Lee’s argument is that it is possible to entrust only nurses with a series of procedures such as pre-determining → examination → judgment → vaccination → observation → first aid, but it is impossible under the current law.

Expert “Each person has a role, expertise”

Kim Woo-joo, a professor of infectious medicine at Korea University Guro Hospital, said, “The medical field such as vaccination consists of a team of doctors and nurses. It is judged by a doctor and administered by a nurse. Each person has their own role and expertise. The role of doctors and nurses is also divided into the guidelines for the corona 19 vaccination of the Agency for Disease Control and Prevention.”

Amendment to the Medical Law

Article 2 of the Medical Law stipulates that doctors are responsible for medical and health guidance, and nurses are required to provide medical assistance under the guidance of doctors, dentists, and oriental doctors. Lee Chang-joon, the health care policy officer of the Ministry of Health and Welfare, said, “It is a violation of the current medical law to completely entrust vaccination to a nurse.” During the national influenza (flu) vaccination last year, doctors and nurses worked as a team. The same goes for the hepatitis B vaccine. If, as Governor Lee argued, would give a nurse both the authority and responsibility for vaccination judgment, the medical law must be revised. It means that it will be blocked from the preliminary stage. Yejin is a practice that is unique to doctors.

Jeong Ki-seok, a professor of respiratory medicine at Hallym University Sungshim Hospital, said, “Yejin must understand the various situations the patient faces. Even when they say’dizzy’, they decide whether to give an injection or not.” “If you can’t make this judgment and return all the patients irresponsibly, it is inefficient. This is what an experienced doctor should do.”

If a problem occurs after the current vaccination, the doctor is responsible if it is judged that the diagnosis is wrong. There is a distribution of responsibilities between doctors and nurses. Moreover, vaccines on new platforms like Pfizer and Modena have reported several times more allergic reactions than traditional vaccines. Yejin is more important.

In the medical world, it is a public opinion that vaccination is by no means a’minor’ medical practice as in this Governor’s article. Lim Hyun-taek, chairman of the Korean Pediatrics and Youth Medical Association, criticized the Governor through his Facebook page. Chairman Lim said, “The minor medical practice results in anaphylaxis (a typical side effect of the vaccine) and dies in less than 30 minutes.”
