Pfizer Vaccine No. 1 Vaccination Center conducted a mock training at the National Medical Center
Recruitment of simulated inoculations and reception → preliminary examination → vaccination → post-vaccination management → response to adverse reactions
“The nationwide plan to inoculate 150,000 people a day…I will improve efficiency based on the training results”

When the announcement of “Calling the quick response team, calling the quick response team” comes out, four doctors are dispatched. Check the patient’s blood pressure, pulse, and breathing, put on an oxygen respirator, and provide first aid. One doctor who judged that the patient was in poor condition asked, “Please call the emergency room for anaphylaxis (there is a patient),” and the waiting nurse picked up the phone and shouted “Patient ambulance moves. Please open the gate.” Six minutes after the rapid response team is dispatched, the patient is loaded into an ambulance and transferred to a nearby emergency room.

Mock training for medical staff to vaccination against the novel coronavirus infection (Corona 19) was conducted at the Central Vaccination Center of the National Medical Center in Seoul for an hour from 2 pm on the 9th. After vaccination, medical staff were trained to cope with a variety of emergency situations, including the occurrence of anaphylaxis, a serious adverse reaction that can result in shock due to a systemic allergic reaction.

The Central Vaccination Center was established earlier this month for Pfizer vaccination that requires cryogenic storage. The government plans to build 250 places nationwide and inoculate 150,000 people a day. If the vaccination center is operated for 600 people per day, 6 hours per day, 100 people per hour and 50 people per 30 minutes should be inoculated. On this day, the training was conducted to recruit 50 mock vaccinations to complete the vaccination within 30 minutes, and to manage after vaccination and cope with adverse reactions. The mock inoculation was also made up of medical staff to perform the vaccination task. To experience the position of the actual inoculator.

Inoculations go through the process of reception, interview, waiting, preliminary examination, vaccination, and post vaccination management. At 2 p.m. that day, the simulated vaccinationists received number tags and entered the building F of the vaccination center. After body temperature measurement and hand sterilization, the identification card was submitted and received. If you fill out a questionnaire asking whether you are pregnant, underlying disease, allergies, or taking medicine, and submit it to the nurse, the nurse will first determine whether or not the vaccine is possible. Inoculation subjects are divided into general vaccination subjects who observe adverse reactions for 15 minutes after vaccination and vaccination caution subjects who require careful observation for 30 minutes.

At 2:8 pm, simulated vaccinationists began to enter the building C, where pre-diagnosis, vaccination, and post vaccination management were performed. If you wait in the waiting room, you will be randomly assigned to 1 out of 4 pre-existing doctors according to the instructions. Ye-jin doctors ask the mock-inoculators “Do you have any disease?” “Is this your first time getting a vaccine?” Check the patient’s history by asking questions such as “You can go home after observing 15 minutes after vaccination”, “The vaccination site may turn red, but it is a normal immune response” “However, if you have serious symptoms after returning home, please call 911” Took the guide.

In the vaccination room in the same space as the Yejin Room, 4 nurses in charge of vaccination and 2 nurses preparing vaccines were waiting. The inoculation room is equipped with a refrigerator to store the thawed Pfizer vaccine, and two clean benches to hygienically separate one person from vaccine vials (glass containers) for 6 people. The nurse put a syringe into the vial and put one person in it. The vaccination nurse took over and gave it to the patient.

The inoculum is transferred to the observation room next to the inoculation room. General inoculations go to the general observation room, sit in a chair for 15 minutes, check their body changes, and return home if there are no adverse reactions. There are a total of 34 seats. After vaccination, subjects are transferred to an intensive observation room for more careful management for 30 minutes. There are 8 beds, not chairs.

At 2:37 pm, an emergency situation was produced. One mock vaccinator served as the patient. When the’fast response team’ is called through the announcement, four doctors are dispatched to move the patient to the intensive care unit next to the observation room and diagnose the symptoms of anaphylaxis. Afterwards, the patient boards an ambulance with the help of a nurse and moves to the emergency room of the National Medical Center’s main office, right next to the vaccination center at 2:43 pm. The emergency room medical staff received a report of an outbreak of anaphylaxis and prepared emergency treatment in advance.

A milder patient outbreak was also produced. One mock inoculum complained, “My head is dizzy.” The doctor said, “There may be temporary dizziness. If you lie down for about 2 to 3 minutes, it will be stable,” he said, and he moved the patient to the intensive care unit. If the patient’s symptoms worsen, seek emergency treatment as in the previous case.

The time when the 50th simulated inoculum completed the vaccination and moved to the observation room was 2:44 pm. It was 14 minutes longer than the 30 minutes targeted by the vaccination center. After the training was over, four people from the vaccination center, including Oh Myung-don, head of the central vaccination center, gathered to discuss problems that need to be improved. Officials decided to increase the size of the observation room while identifying the section where the bottleneck occurs.

Pfizer vaccine is considered the most difficult type of vaccine introduced in Korea because it requires storage and distribution at a cryogenic temperature of -70°C. If not used within 6 hours after exposure to room temperature, it should be discarded. Modena vaccine can be stored at -20℃ and the rest can be stored at room temperature. For this reason, the Vaccination Center explained that the manual obtained from the Pfizer vaccination training on that day could be applied to other vaccinations.

Pfizer’s vaccine will be introduced to Korea at the end of this month or at the beginning of next month with approximately 60,000 people (117,000 doses) through Cobax Facility. The government plans to give the vaccine first to about 50,000 medical professionals who care for corona19 patients.
